What Does a Longevity Health Check Actually Involve?

Updated: Jul 23

The word longevity has become sufficiently fashionable that it risks losing its clinical meaning. There are now longevity supplements, longevity retreats, longevity apps, and longevity coaches, most of which have little to do with the science of extending healthy years of life and a great deal to do with marketing to men who are, understandably, paying more attention to how they age.

Miller Health editorial cover: what a longevity health check involves.

A longevity health check in London, properly understood, is something more specific and more demanding. It is a systematic assessment of the biological systems whose decline determines not just how long you live, but how well you function across the decades that remain. This article explains what that assessment should include, what it reveals, and why it matters for men over 40 who are thinking clearly about the second half of their professional and personal lives.

The Difference Between a Health Check and a Longevity Assessment

A standard health check, whether NHS or private, is designed to detect disease. It asks whether your cholesterol is high enough to warrant a statin, whether your blood pressure indicates hypertension, whether your fasting glucose suggests diabetes. These are important questions. They are not longevity questions.

Longevity medicine asks different questions. It asks how much visceral fat you are carrying around your organs and how that is affecting your insulin sensitivity and your cardiovascular risk over a ten-year horizon. It asks whether your bone density is tracking in a direction that will cause a hip fracture in your sixties. It asks whether the gradual decline in your lean muscle mass is proceeding at a rate that will significantly impair your physical function by your mid-fifties. It asks what your inflammatory markers suggest about your biological age relative to your chronological age.

These are the questions that determine healthspan, defined as the years of healthy, high-function life, not just lifespan measured in heartbeats.

Men Over 40 Health Check: The Diagnostic Stack That Matters

Body Composition via DEXA Scanning

A comprehensive longevity assessment begins with an accurate body composition measurement. DEXA scanning provides segmental lean mass, total and regional fat mass, visceral fat area, and bone mineral density in a single fifteen-minute assessment. These are the foundational metrics. Everything else is interpreted in their context.

Visceral fat area above 100 square centimetres is associated with meaningfully elevated metabolic risk. Lean mass below age-adjusted reference values indicates sarcopenia is underway. A T-score below minus 1.0 on the bone density assessment flags osteopenia requiring attention. These numbers are not academic. They are the biological markers of how you are ageing.

Advanced Metabolic and Cardiovascular Blood Panel

The advanced blood panel in a longevity assessment covers territory the standard panel does not approach. Fasting insulin and HOMA-IR for insulin resistance. Apolipoprotein B for true cardiovascular particle risk. Lipoprotein(a) for the genetic cardiovascular risk factor present in one in five men. High-sensitivity CRP for systemic inflammation. Homocysteine for vascular and neurological risk.

Together with the full hormonal assessment, including free testosterone, SHBG, oestradiol, and thyroid panel, these markers give a comprehensive picture of the biological processes that determine cardiovascular health, metabolic function, and hormonal vitality.

Resting Metabolic Rate Assessment

Measuringresting metabolic rate through indirect calorimetry establishes your precise caloric baseline and your respiratory quotient. For a longevity assessment, the respiratory quotient is particularly informative: it reveals your current metabolic flexibility, specifically whether your body is able to oxidise fat efficiently at rest, a strong marker of metabolic health and a predictor of sustained energy and appetite regulation.

The Peter Attia and Outlive Framework in Clinical Practice

Peter Attia's Outlive, published in 2023, brought a degree of mainstream visibility to the framework of longevity medicine that was previously confined to specialist practice. The book argues that the four horsemen of chronic disease, cardiovascular disease, cancer, neurodegenerative disease, and type 2 diabetes, all have extended preclinical periods during which intervention is possible and effective. The role of a longevity health check is to identify where you sit on those trajectories before symptomatic disease appears.

The diagnostic framework Attia advocates includes many of the markers covered by the Miller HealthExplore assessment: DEXA body composition, comprehensive lipid and metabolic panel including ApoB and fasting insulin, hormone panel, and inflammatory markers. The clinical rationale is consistent with what we have built our assessment around.

Where we add clinical value beyond the biomarker picture is in the nutritional and lifestyle analysis of what is driving those markers and what specific dietary, behavioural, and chronobiological interventions are supported by the evidence.

Longevity Doctor London: What to Look for in Clinical Support

The term longevity doctor is not a regulated clinical title. It is a market descriptor. The quality of clinical support available under that banner varies enormously, from medically supervised and evidentially grounded to commercially driven and clinically superficial.

When evaluating any longevity health service in London, the relevant questions are: What qualifications hold the practitioners to regulatory account? What is the diagnostic methodology? Are the interventions derived from published clinical evidence or from proprietary protocols without an evidence base? Is the assessment personalised to your specific data, or is it a template applied to everyone who comes through the door?

At Miller Health, our practitioners are HCPC-registered dietitians, regulated by the Health and Care Professions Council. Our diagnostic methodology is built around gold-standard measurement tools, DEXA scanning, indirect calorimetry, and a clinically validated blood panel. Our recommendations are grounded in the published literature on metabolic health, hormonal optimisation, and nutritional science.

Quantum Biology and Longevity: The Emerging Science

The field of quantum biology is producing research that will increasingly inform longevity medicine. Work from researchers including Alexander Wunsch on the biological effects of light on mitochondrial function, from Jack Kruse on the relationship between deuterium-depleted water and mitochondrial efficiency, and from Carrie Bennett on circadian biology and metabolic health is building a picture of longevity that extends beyond diet and exercise into the photonic and electromagnetic environment we inhabit.

The clinical integration of these insights is at an earlier stage than the core metabolic and cardiovascular literature. We integrate them into our clinical thinking where the evidence is robust and the practical implications are clear, particularly around circadian alignment, morning light exposure, and time-restricted eating, without overstating what is currently established.

What Happens After a Longevity Health Check in London

The Explore assessment at Miller Health is the diagnostic foundation. It gives you an objective picture of where your key biological systems are today. The clinical report that follows contextualises those findings with specific, prioritised recommendations.

For men who want a structured programme to act on those findings, theEngage programme at £450 per month provides twelve weeks of clinical support: four sessions per month with an HCPC-registered male dietitian who builds and adjusts a personalised protocol based on your data.

For men whose longevity goals require a deeper and more sustained clinical relationship, the Executive programme provides that level of engagement. Speak with us to discuss whether that is the appropriate structure for your situation.

Begin Your Longevity Health Check in London

The best time to assess your biological trajectory is before a diagnosis forces the question. The diagnostic tools exist. The clinical knowledge to interpret them is available. The intervention window for the conditions that will determine how you age in the decades ahead is open now.

The Explore assessment is £1,950 and available at 25 Harley Street, London.
